    Probably because you are grappling to a place where you can't go.

    It's because you threw the grappling hook, then ran into the hook before it hit the ground.

    You should try to grapple a zombie, then right click many times... Fly haxor zombies :V
    It's also a perfect shield in case someone shoots a Barret at you.

    You must have accidentally grappled yourself. Try to let the hook hit the ground before you grapple.
